Stride Physiotherapy Clinic is committed to protecting your personal and personal health information in accordance with Ontario’s Personal Health Information Protection Act (PHIPA) and Canada’s Personal Information Protection and Electronic Documents Act (PIPEDA).

1. Information We Collect

• Contact details (name, address, phone number, email)
• Health information (medical history, assessment notes, treatment plans)
• Billing and insurance information
• Website usage data (see our Cookie Policy)

2. How We Use Your Information

Personal health information is used solely to provide assessment, treatment, and continuity of care, to process billing and insurance claims, and to communicate appointment reminders. We do not sell or rent your personal information to third parties.

3. Disclosure of Information

We only disclose personal health information with your consent, except where required or permitted by law — for example, to other regulated health professionals involved in your care, or when required by a court order or public health authority.

4. Storage & Security

Records are stored securely in PHIPA-compliant electronic health record systems with restricted, role-based access, password protection, and audit logging. Physical records are kept in locked storage.

5. Retention & Disposal

Health records are retained for the period required under Ontario regulation and securely disposed of thereafter using confidential shredding or secure electronic deletion.

6. Your Rights

• Request access to your personal health information
• Request correction of inaccurate information
• Withdraw consent for certain uses or disclosures (subject to legal exceptions)
• File a complaint with our Privacy Officer or the Information and Privacy Commissioner of Ontario
